What we do

If you or your loved one needs professional nursing services to manage severe illness like Covid 19, cancer or trauma or injury or diabetes wounds care, tube or catheter changes, regular injections, bedridden due to illness or in need of selfcare by nurse aides, call 1-800-509-3050 Ext 300 and we will send out Registered Nurse to you. We are nonprofit and licensed by Maryland Department of Health as a Residential Service Agency providing level 3 complex care.

Adult Service Center

Service Descriptions Service $ Charges per Hour

1. Diagnostic/Global Nursing Assessment/Year 100 – 150

2. Treatment/Individualized Care Plan 100 – 150

3. Individual/Family Caregiver Training by a RN 100 - 150

4. Nursing Service by a Registered Nurse 75 – 150

5. Nursing Service by a Licensed Practical Nurse 60 – 120

6. Nursing Service by a Nurse Practitioner 150 – 200

7. Nursing Service by a CNA/GNA/CMT/RN Supervision 25 – 35

8. Live-in Aide by a CNA/GNA/CMT/RN Supervision 180 – 240/day, (not working all hours but present)

9. Live-in Aide by a CNA/GNA/CMT/RN Supervision 360 – 480/day (three shifts)

Difference in price depends on the professional skills needed, the assessment that must be performed by an RN, individualized care plan, individual’s choices and preferences, staff performing the needed procedures, equipment and supplies needed by our agency staff, distance of travel to provide needed care, regulatory requirements for specific care, and availability of insurance coverage or third-party payer for the needed care. All services start with individualized nursing assessment and care plan which shall be completed by an RN.

A clinical evaluation must indicates that the individual has a primary diagnosis and severe medical/mental problem which is the cause of significant physical, psychological, personal care, vocational, education, and/or social impairment.

At the Holy Trinity Center, we treat people starting with an assessment: Wound and catheter care, Treatment of stage three or stage four skin ulcers, Ventilator services, Skilled monitoring, testing, and aggressive adjustment of medications and treatments where there is the presence of, or risk for, a fluctuating acute condition; Monitoring of a chronic medical condition that is not readily controllable through available medications and treatments; Infusion therapy; Specialized intravenous therapies or nutrition support; Monitoring for being at high risk for health or safety complications which cannot be adequately managed; or A different level of care after notification of a significant change of condition; an assessment of an individual served, and referral if needed; Contact us at www.holytrinitycenter.org
