Holy Trinity Family Support Center has solution for some of your children, adult, and family severe medical health, human development, health education, civic education, and many independent training needs. This is a Non-Profit 501 (C) (3) organization. We are also licensed as a residential service agency by the Maryland Department of Health, Office of Health Care Quality, to provide Skilled Nursing and Aides; Level Three: Complex Care Provided by a Registered Nurse (RN)/Licensed Practical Nurse and RN Supervision of Aides.

We are accepting application to our Children Service Center Programs. We treat, rehabilitate, teach, coach, educate, train, and mentor children on variety of issues and developmental challenges. There are children being forced in marriage, sex-slave, and teen pregnancy which affects their physical and emotional development. Children run around in communities without adequate parental supervision and direction. The parental practice of Natural Growth child rearing has taken dominance over the practice of Concerted Cultivation among middle class, lower class, working Americans and poor people in the United States; causing breakdown in American families. Elochukwu, (2012).

There are children who are basically raising themselves with whatever survival tactics or techniques they pick up from the streets. Gangs and gang related activities are at alarming rate. Children engage and particularly suffer violence in street fighting, truancy, selling drugs, stealing, homicide, suicide, and crimes against human dignity; such as rapes, sexual, emotional and psychological abuses, neglects, domestic violence, gang fights, bullying, assault, aggressive attacks, subhuman living conditions, homelessness, unjust imprisonment, prostitution, and violence in conflict, neglect, and other abuses. Lack of opportunity, inequality, injustice, and discrimination persists.

We provide opportunity to address these concerns in variety of ways:

We believe that every child has the right to appropriate individual development, including the ability to make decisions about the individual’s life and create opportunities for choice regarding home, school or work, and community. Children have right to effective and efficient use of community resources for self-integration within their immediate surroundings and community. Children has the right to strong educational development, skills, and employment development from their foundational level, and nurturing assistance, care, and love.

Our support services also include information sourcing and sharing, guidance and counseling, outreach services, health care interventions, educational support, health education and awareness, support to bridge lack of access to entitlements and resources, cultural support, ethnic and socio-economic barriers, stigma removal and orientation, mal-adaptive behavior intervention, behavioral health, advocacy, solicitation, prevention services, and family preservation. We do more than can be described here.

We guarantee progressive improvement within three (3) months in all our programs. Your registration fee is for this program. You can register for more than one program. Each program is registered separately. Three months commitment is required to participate in this program. Registration can be renewed on monthly basis.

Participants are engaged and parents in particular encouraged to support different evidence-based practices used by staff and volunteers to serve our children. We have several social, leisure, recreational, and interpersonal activities that stimulate age appropriate development.

Service Descriptions Service $ Charges per Hour

1. Diagnostic/Global Nursing Assessment/Year 100 – 150

2. Treatment/Individualized Care Plan 100 – 150

3. Individual/Family Caregiver Training by a RN 100 - 150

4. Nursing Service by a Registered Nurse 75 – 150

5. Nursing Service by a Licensed Practical Nurse 60 – 120

6. Nursing Service by a Nurse Practitioner 150 – 200

7. Nursing Service by a CNA/GNA/CMT/RN Supervision 25 – 35

8. Live-in Aide by a CNA/GNA/CMT/RN Supervision 180 – 240/day, (not working all hours but present)

9. Live-in Aide by a CNA/GNA/CMT/RN Supervision 360 – 480/day (three shifts)

Difference in price depends on the professional skills needed, the assessment that must be performed by an RN, individualized care plan, individual’s choices and preferences, staff performing the needed procedures, equipment and supplies needed by our agency staff, distance of travel to provide needed care, regulatory requirements for specific care, and availability of insurance coverage or third-party payer for the needed care. All services start with individualized nursing assessment and care plan which shall be completed by an RN.

A clinical evaluation must indicates that the individual has a primary diagnosis and severe medical/mental problem which is the cause of significant physical, psychological, personal care, vocational, education, and/or social impairment.
